چکیده

For thoraco-abdominal aortic aneurysms (TAAA), it is unclear whether better to perform hybrid repair in one (single) or two stages (staged). This study aimed compare the clinical outcomes of single vs. staged TAAA. The Medline, Embase, and Cochrane Databases (1 January 1994 11 May 2020) were searched for studies on Cohort case series reporting TAAA eligible inclusion. Newcastle–Ottawa scale an 18 item tool used assess risk bias. primary outcome was 30 day mortality, secondary included post-operative complications, overall survival, other mid term events. A random effects model calculate pooled estimates. total 37 meta-analysis. quality assessment suggested low moderate estimates aneurysm rupture death during stage interval 2% (95% CI 0%–4%, I2 = 0%) 4% 2%–7%, 0%), respectively. Single associated with a significantly higher when compared patients who completed procedures successfully (OR 2.64, 95% 1.36–5.12, 0%). Staged also had lower incidence major adverse cardiac events (MACE) (single: 10%, 5%–16%; staged: 2%, 0%–5%) intestinal complications 15%, 8%–25%; 3%, 1%–6%). outcomes, comparable 12 month related rate re-intervention, graft patency. Two may represent choice controlled rupture, because can provide mortality risks, MACE, as well outcomes. Randomised trials are needed ascertain effect staging elective

Transabdominal repair of type IV thoraco-abdominal aortic aneurysms.

Aneurysms that involve the supracoeliac and/or descending thoracic aorta have been repaired through a left thoraco-abdominal incision (Fig. 1). While this affords excellent exposure of the entire thoracoabdominal aorta, ~ it necessitates division of the left costal margin and at least partial transection of the left hemidiaphragm to permit adequate separation of the ribs. Both peritoneal and pl...

The assessment of collateral communication after hybrid repair for Crawford extent II thoraco-abdominal aortic aneurysms.

OBJECTIVES The repair of extensive thoraco-abdominal aortic aneurysms (TAAAs) is invasive and carries a high risk for spinal cord injury (SCI). The aim of this study was to assess the early results and collateral circulation to the spinal cord after hybrid repair for Crawford extent II aortic aneurysms. METHODS Between 1997 and 2013, we performed 128 thoracic endovascular aortic repair (TEVAR...

[Efficacy comparison between endovascular aneurysm repair versus open surgery for patients with abdominal aortic aneurysms: a meta-analysis].

OBJECTIVE To compare the efficacy between endovascular aneurysm repair versus open surgery in patients with abdominal aortic aneurysms (AAA). METHODS A systematic review was performed to identify clinical outcomes of randomized controlled trials for AAA patients receiving endovascular aneurysm repair or open surgery. The Cochrane Library (issue 7 of 2011), MEDLINE (1996 to 2011), EMBASE (1974...

Current results of endovascular repair of thoraco-abdominal aneurysms†.

OBJECTIVES Fenestrated and branch endografts represent a totally endovascular solution for high-risk patients with atherosclerotic thoraco-abdominal aortic aneurysms (TAAAs). This study reports the early outcome of endovascular TAAA repair. METHODS Interrogation of a prospective database of consecutive patients who underwent endovascular repair (EVAR) for TAAA between June 2007 and October 20...

Retroperitoneal versus transperitoneal approach for repair of abdominal aortic aneurysms.

We, as well as other authors, believe that the retroperitoneal approach is an excellent alternative to the transperitoneal route for the repair of abdominal aortic aneurysms. This approach is associated with a significant decrease in pulmonary and cardiac complications and therefore can be used in selected high-risk patients with expanding aneurysms. A well-controlled randomized multicenter tri...
